Dazzy Vance Twice-Signed Handwritten Questionnaire (PSA)
Questionnaire, apparently from the Brooklyn Dodgers, signed twice by Hall of Fame hurler Dazzy Vance. Typewritten mimeograph copied form measures 7.75x7.5" and has been filled out in blue ballpoint ink, all in Vance's hand. That includes item #1 (Player's Name) which is signed, A.C. "Dazzy" Vance, and in item #14 (Nicknames) he has again written "Dazzy." While the form is undated, some clues can be found in question #8 (Present Occupation), which he lists as Retired; item #9 (Uniform No.) which he lists as 15 - he only wore this number for the Dodgers; and item #3 (Years With Club) in which he has written 12, the number of years in which he played for Brooklyn's Bums. Some folds are evident as is tape residue at each of the four corners, slight foxing at the edges, back is blank. Encapsulated by PSA/DNA as Authentic Auto, comes with LOA.
